*Home care memberships are accepted on a case-by-case basis, as there is limited availability for this service. A home-care membership is one during which more than half of patient encounters take place in the patient’s place of residence or assisted-living facility. Note: Rates quoted above are based on a 12-month agreement. Services billed monthly and quarterly are paid via an automatic payment method. Patients who do not agree to automatic payments accept a 20% increase of each total invoice amount for additional billing processing. A three-month, non-refundable commitment fee equal to and counting toward the first three months of membership due upfront and in full at the time of registration.
