Daisy Family Health Care follows the Direct Primary Care Model. Direct Primary Care is a membership-based primary care agreement between a patient and their provider. The member pays a monthly membership, and in return gets excellent primary care. These primary care services include whatever you would normally go to your regular primary care provider for, unlimited. Patient + Provider = Direct Primary Care No CoPays - No Deductibles

Pricing details

Includes all office visits, wellness, sick visits, follow up, disease management, telemedicine. Labs included once per year, other labs are discounted. Hormone Therapy, Vitamin Infusions, Medication and so much more are also discounted.
