Family Memberships: The 3rd, 4th, and 5th children are free. Children/Step-children can be included in family plans up to age 26. 6th child and beyond incurs $75/child/month fee

Dr. Moak-Blest was born and raised in Newburyport, MA. She completed medical school in Florida and returned to MA for residency training at UMass Worcester. She has worked in the north shore for over ten years and has returned to practice in her hometown where she is currently raising her family
