Western Illinois Home Provider Care started in 2024 as an additional service line to Western Illinois Home Health Care, a family-owned and operated home health care agency serving ten counties throughout West Central Illinois. Home Provider Care offers primary care, telemedicine, sick visits, advanced wound care, transitional care, and chronic disease management to wherever patients call home. HPC offers a hybrid model accepting both insurance and an innovative DPC model.

Membership benefits include: Priority scheduling, discounted labs, imaging, and prescriptions. Annual comprehensive wellness visit for each member with included lab panel. 4-6 telehealth visits and 8-10 in-home visits depending on household size.
