Healthy Adult Plan: Over aged 35 with 2 or less medications? $55/month with yearly physical included and 4 office visits. AGES 66-100 WAITING LIST PLEASE CONTACT US **$165/MONTH FOR A NUCLEAR FAMILY UP TO 4. $10 ADDITIONAL FOR EACH EXTRA PERSON IN THE HOME.

Stephanie is a board certified Family Nurse Practitioner with the American Academy of Nurse Practitioners. She also holds a Doctorate degree in Nursing Practice. She is a native of Ashe County and has called Wilkes County home for the past 15 years.
